Used this to install my older 1.125" Reba 29 SL on a new carbon frame that is 1.5" and it works great. Matches perfectly with the headset that cam with the frame. On the mountain there is no flex in the headset that I can tell. Great solution to allow me to keep using the older fork.

If you have a 1 1/8" straight fork that you want to use on a tapered 1.5 -> 1.125 frame, all you have to do is use this in combination with any Cane Creek headset.
I do not know if it would work with any other headset, but the Cane Creek 40 is the standard for an inexpensive, high-quality headset.
